Equipment Register
The operator shall maintain an up to date register of all playground equipment.
The register should include:
- The location of the playground
- Location of the equipment within the playground
- Equipment & under surfacing details
- Installation dates
- Installer details
- Manufacturer warranty details
- Certificates of inspections and testing (if necessary)
- Inspection and maintenance instructions
- Operating instructions, if applicable

Inspections
The operator shall develop and maintain a program of inspections for all items of playground equipment based on the manufactures instructions and local risk factors. The program shall include the frequency of inspection, and the elements to be inspected. Where hazards are identified, corrective action shall be initiated.
Corrective Action
The operator shall develop and maintain procedures for corrective action where hazards are identified for example by inspection, in incident reports, or by other people notifying the operator. Corrective action includes measures to be taken to correct defects or to reinstate the safe operation of the playground. Any defect which puts safety at risk shall either be corrected without the delay, or the item shall be secured against use or removed. Care should be taken that repairs are not carried out in a manner which creates an additional hazard.
Maintenance Procedures
Operators of playgrounds shall develop and maintain maintenance programs for all items of playground equipment based on the manufactures instructions and local risk factors.
Incidents
Operators of playgrounds shall develop and maintain procedures to be followed in the event of injuries, near misses and equipment damage. All information about incidents brought to the attention of the operator should be recorded on a form which includes at least the following details:
- Date & Time of incident
- Weather Conditions
- Location of incident
- Description of incident
- Equipment involved (if any)
- Contributing factors
- Corrective action recommended
- Date corrective action completed
If the incident involved personal injury, the following information should also be recorded:
- The injured person/s details, including name, address, age and gender
- The injury sustained
- The person(s) notified, and the time and date of notification
- Treatment (if known)

Records
